What is hospice care?
Hospice is care for people with a serious illness who are no longer seeking treatment to cure it. The focus is on comfort, quality of life, and support for the whole family.
Who is eligible for hospice?
Under Medicare, a patient is eligible when their physician and the hospice medical director certify a life expectancy of six months or less if the illness runs its usual course. Patients can continue to receive hospice care beyond six months as long as they remain eligible.
Where is care provided?
Most hospice care happens wherever the patient lives: a private home, an assisted living community, or a skilled nursing facility. Call us to talk about your situation.
Can my loved one keep their own doctor?
Yes. Patients may keep their own physician, who can work together with our hospice team.
What if my loved one’s condition improves?
If a patient’s health improves or they decide to resume curative treatment, they can leave hospice care. They can return later if they become eligible again.
Do you provide support after a loss?
Yes. Our bereavement team provides grief support to families for at least a year after a patient dies.
